    Hello hello October Batch.. Tanong lang.. May nagsubmit ba sanyo ng payment summary from employer instead of payslip and ITR? Wala na kasi akong copy ng payslip and ITR sa previous employer/company ko. Naisip ko lang na baka pwedeng gawing alternative ung summary statement from employer.

    @nayabrayaj yung payment summary ba sir is from your company, with logo/header and signed by an authorized entity (HR)? If yes, it would help but I cannot guarantee if satisfied na sya dun. In my case, I provided other 'proof of employment' like contract of car plan, promotion, etc but backed up by ITR and random payslips.

    @cp101030 dpat yung name sa lahat ng docs same. kung ano yung nsa passport, yun na sa lahat pra consistent. ako recently married but but di ko pa cinarry surname ni hubby hanggang ma ok lahat ng docs kasi hassle maxado. di din nmn uso dito na i carry agad ang surname ng hubby. otherwise if married name na yung s passport, mg stat dec nlng kayo explaining the circumstance bakit iba yung surname sa CoE

    @cp101030 kung nag change na ng name si wifey mag provide kayo ng deed poll document (change of name). Makakakuha ka nyan sa phil embassy sa singapore.

    Ganun ginawa ng kakilala ko. 10years experience ng wife nya tapos sa last two years experience lang nagbago ang surname. kumuha sila ng bagong COE sa mga previous company stating the new married name ng babae. Sa mga form 80 mag fifill out ka rin ng is the applicant known by other names.

    So better provide evidence ng change of name like deed poll.

    @cp101030 Not sure how the ACS application goes but sa visa sa ImmiAccount there are options to indicate other names you were known by. I indicated my maiden name dun (bec hindi pa ako tapos mag update lahat) ang chose the reason na due to marriage. Also, she will always have docs under maiden name so better indicate it on all, if not most, forms :-) (Think birth cert, old passports, school documents)
