    Base sa mga nabasa ko dito sa forum, may mga naglagay ng middle name ( under sa given name field) at meron din naman na hindi.
    Both approach worked with no issues - basta lang be consistent all throughout the application and forms submitted.
    Ako naglagay ako para lang sumakto sa statement na “as it appears in your passport” - pero sabi nga ng iba dito na may mga visa grant na din na di naglagay ng middle name e ayos lang din kahit wala. So it is up to you kung san ka mas kampante :)

    @BrizyFilo Hindi ko nilagay middle name ko sa Given Names filed. Naka-DG naman ako. So up to you kung gusto mo ilagay o hindi.

    Hi po, I have a question, ung husband ko po ung main applicant, after graduation nag home base xa sa Pinas for 9 months, pero wala pong mga government docs like BIR. sulat din po ba to sa employee details sa form 80? d naman na declare din sa ACS.
    Gradute xa ng march tas freelance from april to dec.

    baka may same situation po dito. Thank you.

    Pagkakaalam ko yung work history in the past 10 years, sa EOI and assessment lang yon. Sa Form 80, AFAIK all employments/ non-employments since birth.

    I'd usually say just be honest and declare it. But...kung ako yan, I'd just say unemployed/supported by family during that period. It's very normal for fresh grads to take a break after studying. :D

    @ktelita yes po, kailangang isulat. I was unemployed ng 2 years, nilagay ko sa form 80 ko. Kailangan kasi walang gap.

    Just wanted to confirm lang po, did you include your middle name as part of your given name? Especially sa married women? Medyo mixed kasi yung responses na nababasa ko regarding this. In our EOI and assessments (PTE & ACS) walang middle name. I was wondering if okay lang ba na walang middle name for Form 80? Is it necessary to include it? Thanks in advance :)

    Ako rin litong lito na dito haha.
    In our EOI, ACS assessment and PTE, we did not include middle name.
    Am still filling out the visa application form, form 80 and form 1221. Di ko pa rin alam kung isasama ko middle name.
    Tapos sa form 1221 question 14, nakalagay "name shown in document". Isama ba middle name or not?

    Additional info: when i applied for temporary visa last 2010 para sa business trip, wala rin naman ako middle name kahit sa grant. Pero syempre iba nga naman yung PR visa hehe. Baka lang may ibang rules.

    @crankygrinch In my opinion, it would be safe to maintain consistency in documentation. It is true that some documents request that middles names be included while some would not. However, during my first entry in Au, I was questioned at immigration because my visa does not indicate my middle name. I explained the matter to the officer and she suggested that next time I encode everything as shown in my primary identification document which in my case is my passport.

    For ITA, yes only those assessed by assessing authority but for Form 80, everything because it asks that there be no gaps in employment history. Even those periods of unemployment must be explained.

    Sa Q17 po ba countries resided for last 10yrs, kailangan po ba ilagay ung address ko sa Malaysia althought 5 months cumulative lang yung duration of stay?

    Nagrrquest kasi CO ng form 80 and PC from malaysia. Kaso to get PC from malaysia you need 1 yr stay sa malaysia. Now, confused ako kung ilalagay ko yung malaysia sa Q17.

    @aron_drn kelan po kayo kumuha ng malaysia PC?

    Worry ko kasi baka umabot ng 1 month bago marelease yung Malaysia PC and nakalagay naman dun sa CO checklist na "Provide Malaysia Police Check If you have lived in Malaysia for 12 months or more in last 10 years"

    If magsubmit kami ng form 80 and we will include Malaysia as residence, makikita naman nila na less than 12 months of stay.. Do we still need to provide the Malaysia police clearance.
