Should I include Middle Name in lodge

Mga kaibigan, nalilito po ako kung ilalagay ko ba ang middle name sa paglolodge ng visa information. May iba nagsusuggest na ilagay ang middle name with given name. I also read na from other discussion na middle name in AU is second name and not maiden name. Nalilito na po talaga ako tagal ko na sana ilolodge application ko almost a month na yun EOI ko, as much as I want to send this natatakot naman ako baka mareject at maTY sa visa fee.. OH NO!!! yun ACS ko kc at EOI walang middle name, kung ilalagay ko ba sa visa lodge ko may middle name sa given name ok lang ba yun? baka kc maging invalid kc yun document ko like ACS and EOI walang middle name pero kung d ko naman sya ilalagay baka anman d ako papasukin sa AU dahil yun passport ko may middle name? Please enlightened me. salamat po God bless =)

    Since @TotoyOZresident and @JCsantos advise me to add it, I'll do the same. Thanks sa info mahirap na magkamali malaki halaga din ang stake kaya better ask than be sorry. Salamat Po sana malodge ko na today baka magbago pa policy ni DIAC sana Humana ang AU dollar. :D

    Mga sir, ask ko lang po kung tingin niyo na better po na wag ko nang ilagay sa form 80 and 1221 yung middle name po namin para po consistent sa diac form namin na di po namin nilagay yung middle name? Salamatbpo.

    @bluemist, I made the same mistake as you did. I called DIAC and they asked me to submit Form 1023 and emailed it to a certain email address. You can call DIAC if you would like them to change it for you.

    @bluemist..it doesn't matter po kng isama nyo ang middle name nyo or hndi.. ang importante po ay ang last name at frst name.. kasi sakn nung nag apply ako ng visa hndi ko sinama mddle name ko which is the name of my mum pro sa passport q my middle name.. wla namn naging prblma.. ang tinitngnan namn nla is ung last at frst name lang.
    @nfronda, nilagay ko din dito ang reply mo sa kabila. I agree na ang importante talaga is yung last name at first name. Although pwede din naman isama yung middle name kung gusto nyo. In our case, 3 kami sa visa application, di namin isinama yung middle name , okay naman.

    So 4 everyone applying for a visa don't think too hard po kng e include or not ang middle name.. ang importnte ay ang first and last name..

    06-01-11: Arrived Australia (457 visa dependent on my dad)
    28-05-12: Lodged RSMS 857 visa (seperate from my dad already)
    28-06-12: Done Medical check
    30-10-12: Submit AFP check
    07-11-12: Went home on a bridging visa B for holiday and wedding..:)

    18-01-13: CO asked documents of my husband to be included on my application as migrating dependent..Thank u LORD!

    Sorry to relive this thread. The thing is be consistent here right? My problem is sa assessment ko kasama middle name. Dun sa EOI first name and surname lang. After reading all these, I think I have to include middle name kase yun ang nasa passport ko and sa mga assessments. Do you guys think it will be a problem because yung sa EOI ko does not have a middle name? And I am about to apply na so locked na yung EOI.

    walang issue kung walang middle name. as long as first and last name same as the one in passport

    Hello po, alam ko po na discuss na ito dati. Pero confirm ko lang po ulit kung magiging okay naman sa initial entry kahit di naka include yung middle name/maiden name sa visa grant letter? Baka meron po naka experience na iallow naman ng immigration authorities. Thanks po in advance :) pasensya na po kung makulit, nagsisigurado lang po. Salamat ulit

    @chi1234 include your middle name as per your passport and at the same time, advice your case officer how middle names are used in Philippines. I have this problem now. My mother's name is now part of my first and second name on my Australian passport. When I got married and wanted to use my husband's name, I could easily drop my maiden name but not my mother's maiden name. I have file a change of name if I want to officially drop my mother's maiden name.

    Hello po ako rin di ko nalagay sa eoi ko yung middle name before ako mainvite. Eh locked na siya. Now If ilagay ko po yung middle name sa visa application magkaka-issue po kaya? All other documents ko po kasi indicated ang middle name ko. Sabi po sa thread na to ok lang, pero gusto ko lang iupdate lalo yung inquiry 2013-2014 pa. Baka po may recently granted na nagkaproblem ng ganito?

    @ramaineffr its ok kung nainvite ka na. Naka lodge ka na, be consistent na lang all through out. It really doesn’t matter naman ang middle name natin, importante ang last name at first name/given name. Sa VEVO account, hindi nila nilalagay ang middle name and even sa grant letter..

    No need to worry, you will be fine.

    @ramaineffr the form asks you to put your name as shown on passport. So I make it a point to include all the names on passport for our clients. Never assume that it’s ok because if a case officer questions it, you lose any argument you can think of but if you include everything, then you’ll be more confident you did the right thing.

    I always tell my staff, dealing with the whole application process, you should never assume things. Always be by the book so you’ll never be wrong. Hope that helps.

I just want to confirm the correct format as I am really confused about how I should put it in form 80 and form 1221. I have 2 given names. Should I put my middle name after my second given name?     Middle name usually goes in the Given Name field if the form doesn't have a separate field for it.

    My US visa has my 2 given names + middle name in the Given Name field.

    My AU tourist visas also has the given names + middle name but the PR one doesn't.

    Based sa naaalala ko from similar discussions dito sa forum, wala talaga ang middle name sa grant? When I was granted a tourist visa, wala middle name ko dun and okay naman.
